To qualify, the applicant must have a cut-off notice or disconnect notice from the utility company. The applicant must bring a copy of the cut-off or disconnect bill to the appointment. To schedule an appointment, call 918-582-1356 on Monday or Tuesday between 9:00 AM and 10:30 AM. At the appointment, the applicant must present a photo ID and Social Security card.
